A Mental Health Advance Directive is a document that allows you to make your choices known regarding mental health treatment in advance of a time when you may find yourself in a crisis or involuntarily committed and unable to make decisions. Many decisions may be made on your behalf if you are admitted to a mental health facility during a crisis, such as the choice of hospital, types of treatment and medications prescribed and who should be notified. One way to be certain that your doctors,treatment team and significant others understand your treatment preferences is to have already prepared a Mental Health Advance Directive (MHAD), a legal document when completed properly.
